Dr. Hoffman is in-network with Anthem/BCBS health insurance policies.
For all other policies, the standard self-pay rates listed below apply. Dr. Hoffman can provide you with a superbill to submit to your insurance for out-of-network benefits. Your plan may reimburse you part of the out-of-pocket fee.
Please note that in order to bill services through insurance, a diagnosis/diagnostic impression must be given to describe the main focus of our treatment. If you or your child does not meet criteria for a clinical diagnosis, but therapy could still be beneficial, the out-of-pocket fees will apply.
It is strongly recommended you consult and verify with your insurance company to determine whether services will be considered in-network or out-of-network and your related financial responsibility.
Please note all fees are due at the time of service. This includes payments made toward deductible, co-pays, or co-insurance.
